When someone contacts Indiana Community Addiction Network via our website submission form, we collect their name, phone number, and email address.  However, this online privacy policy applies only to information collected through our site and not to information collected offline, including telephonically, via email, or through any other means. We only have access to/collect information that you voluntarily give us via email or other direct contact from you. We will not sell or rent this information to anyone.

We will use your information to respond to you regarding the reason you contacted us. We will not share your information with any third party outside of our organization, other than as necessary to fulfill your request, e.g. respond to inquiry questions.

Indiana Community Addiction Network is a private substance use and mental health treatment center.  We only provide treatment to individuals 18 years of age or older.  As such, our website is not structured to attract anyone under the age of 18, nor do we knowingly collect any information from individuals under the age of 18.

Indiana Community Addiction Network understands that your personal and medical information are personal. That’s why we comply with the following privacy practices. You will receive a more detailed privacy notice upon enrollment in our program. How We May Use or Disclose Medical Information About You For Treatment: Your health information may be used by staff members or disclosed to other health care professionals for the purpose of evaluating your health, diagnosing medical conditions, and providing treatment. For Payment: Your health information may be used to seek payment from your insurance company(s) (health, dental, auto or other) and from any other source that you may use to pay for services such as credit card, cashier check, personal check and or banking company(s). For Health Care Operations: Your health information may be used as necessary to support the day-to-day activities and management of Indiana Community Addiction Network. Law Enforcement: Your health information may be disclosed to law enforcement agencies, without your permission, to support government audits and inspections, to facilitate law-enforcement investigations, and to comply with government mandated reporting. Public Health Reporting: Your health information may be disclosed to public health agencies as required by law. Other uses and disclosures require your authorization. Disclosure of your health information or its use for any purpose other than those listed above requires your specific written authorization. If you change your mind after authorizing a use or disclosure of your information you may submit a written revocation of the authorization. Payment reminders: Our staff will use your health information to send you payment reminders.

If you believe that your privacy rights have been violated, you should call the matter to our attention by sending a letter describing the cause of your concern to the same address.  You will not be penalized or otherwise retaliated against for filing a complaint.
